Abstract

Abdominal sepsis in oncologic patients after extensive abdominal surgery is associated with high mortality due to uncontrolled systemic inflammation, endotoxemia, and progressive multiple organ dysfunction despite standard intensive care. This study reports several clinical cases in which extracorporeal blood purification using Efferon hemoadsorption columns (Efferon CT and/or Efferon LPS, based on hyper‑cross‑linked styrene–divinylbenzene copolymer with selective binding of cytokines and Gram‑negative endotoxin) was integrated into complex therapy for postoperative abdominal sepsis. Hemoadsorption sessions were performed in addition to conventional source control, antimicrobial therapy, organ support and intensive monitoring. Across the described cases, one or a few procedures were followed by decreases in leukocytosis, endotoxin activity, pro‑inflammatory cytokines and procalcitonin, improvement of hemodynamics with reduced vasopressor requirements, and better renal and respiratory function, with stabilization of SOFA scores. The authors conclude that Efferon‑based selective hemoadsorption is feasible and safe in this high‑risk population, provides rapid correction of key markers of systemic inflammation and organ dysfunction, and warrants further prospective controlled studies to define its role in the management of abdominal sepsis in oncology patients.
